• 76% of the project participants will be discharged to permanent housing (slightly higher than the minimum based on current performance)
• Less than 20% of the exits from the program will be negative, defined as those exits for a violation of program rules, failure to comply with program requirements, or leaving the program without consulting staff. This is slightly lower than the minimum and may be even lower as we have lessened the demands in the project.
• 25% of project participants will increase income or non-cash benefits while in the program. (Many more are expected to increase their income after moving into permanent housing.)
• 90% of the project participants will be placed in permanent housing in 90 days or less
• 100% of the homeless Veterans will be educated on the crisis hotline and services available thru the VA and COV in order for the Veteran to have the information to proactively respond to crisis after exit from the program.
• 100% of homeless Veterans in the program will be assessed to ensure they are receiving the VA and other government benefits to which they are entitled
