The Transition Program
The Transition Program is a “bridge” housing and referral program, that provides many services to the Veteran with an emphasis on permanent housing placement within a 60 day time period. The program provides peer mentoring, case management and vocational rehab. Our therapists, interns and other service providers assist in crisis intervention and stabilization services, assisting the client in achieving goals for independence.
COV provides transportation or transportation vouchers to contracted facilities, VA medical appointments, job and housing search efforts, or other needs identified by the Veteran. The Transitions Program provides needed services to enable the client to perform daily living activities including budgeting, meal planning, personal care, financial services, medical or other healthcare, assistive technology assessment, employment or, other identified needs.
Transition assures that the Veteran’s nutritional and hygiene needs are attended to by providing 3 meals per day 7 days a week, that follow federal nutritional guidelines, and toiletries. As well as, community sponsored holiday meals.
